New Clients
Once we process your portal application, you can book an appointment. You will then receive an intake form and consent form.
These forms must be completed at least 5 full business days prior to your New Patient Medical Psychiatric Evaluation appointment.
Your appointment cannot happen unless this is completed. If these forms aren’t completed within that time frame the appointment will have to be rescheduled.
All prospective Clients should begin by visiting the client portal and requesting portal access. Once signed in, you can book an appointment or request more information.

Fee Schedule & Insurance Info
For Colorado patients, Crestone Mental Health works with Optum, Oxford, Blue Cross Blue Shield of Colorado, Oscar Health, United, Aetna, Cigna and Medicaid.

For Hawaii patients, Crestone Mental Health works with HMSA, Quest, Alohacare, Ohana Care, Optum, Aetna and Cigna.

We require a credit card on file for all patients to ensure we are paid in the event of a no-show or late-cancellation appointment. As per the consent for treatment you’ll sign when you become a patient, we require 2 full business days notice for canceled appointments and appointment changes. Those canceled with less than 2 business days notice or no-shows (more than 15 minutes late for your appointment) will be charged 50% of total price.

Psychiatric Evaluation:
This appointment includes review of records and coordination of care with other providers as needed, Psychiatric Evaluation Q & A as needed, lab/medication assessment as needed.
60-90 minute appointment $250
Follow Up Appointment
This appointment is a combination of medication management and therapy. This appointment will include a review of medication, symptoms, medication usefulness, evaluation for side effects, determination of medication adjustments, review of lab values as needed, supplementation, Q & A as needed, therapy, skills, and skills training/evaluation.
30 minute appointment $100
