Crestone Mental Health offers full comprehensive psychiatric evaluations, medication management and counseling.
What to expect

Your first appointment will last 60-90 minutes. This will include a full comprehensive psychiatric evaluation.

After your initial psychiatric evaluation, you will have medication management appointments. These range from 15-30 minutes and often include therapy.

I treat patients across the lifespan-children, adolescents, adults and geriatric populations. Appointments at this time are only offered via tele-health.

I hope to partner with patients who are ready to be proactive in their healing. Medications can be a great tool to alleviate symptoms, but works best when combined with therapy and lifestyle changes.
Fee Schedule & Insurance Info
For Colorado patients, Crestone Mental accepts cash pay, Aetna, Optum, Cigna, Oxford, Oscar Health, United, Athem Blue cross Blue Shield of Colorado and Medicaid.
For Hawaii Patients, Crestone Mental Health accepts HMSA, Optum, Aetna and Cigna.
We hope to have the ability to bill insurance soon.
We require a credit card on file for all patients to ensure we are paid in the event of a no-show or late-cancellation appointment. As per the consent for treatment you’ll sign when you become a patient, we require 2 full business days notice for canceled appointments and appointment changes. Those canceled with less than 2 business days notice or no-shows (more than 10 minutes late for your appointment) will be charged 50% of total price.
Psychiatric Evaluation:
This appointment includes review of records and coordination of care with other providers as needed, Psychiatric Evaluation Q & A as needed, lab/medication assessment as needed.

Follow Up Appointment
Medication Management patients only (Combination Medication Management and Therapy):
This appointment includes review of medication, symptoms, medication usefulness, evaluation for side effects, determination of medication adjustments, review of lab values as needed, supplementation, Q & A as needed, therapy, skills, and skills training/evaluation.
30 minute appointment $100
